High Plains to Form Patient Advisory Council

High Plains Community Health Center is adding an opportunity for patients and families to provide feedback about their patient experience. Applications are currently being accepted from active patients for membership on a Patient & Family Advisory Council. We want to make sure that our patient’s point of view, perspective and experience are not only heard, but also integrated into our services. The Council meetings will be for an hour each month and start in January 2019.
Applications can be picked up and returned to the greeter at each of our five sites. We want to hear from you!
The sites are:
Main – 201 Kendall, Lamar
Family/Dental – 200 Kendall, Lamar
LCC – 2401 S. Main, Lamar
Wiley – 301 Main, Wiley
Holly – 410 W Colorado, Holly
